EMPLOYEE of the YEAR Nomination Form

Purpose of the Program:
The purpose of the Employee of the Year program is to recognize employees who have best served LCAA. The nominee must be an employee who performs outstanding service through his/her work and exhibits a positive and supportive attitude.

Eligibility for Employee of the Year:
All employees who have been employed for at least one year are eligible. (Excluding Program Directors)

Criteria for selection:
Using the following criteria, please explain why this employee should be considered for Employee of the Year.

1)	Believes in the overall mission and vision of LCAA


2)	Demonstrates outstanding work performance/Productivity


3)	Projects a warm and positive attitude towards others


4)	Team oriented


5)	Demonstrates good attendance
