Energy Assistance Customers!

Before downloading the application for Energy Assistance Please Read all instructions carefully!

Please call the Lake Community Action Agency LIHEAP Appointment Line for the Service Center nearest you on the following days to schedule an appointment to receive an application by mail or apply through our Online application. Applications for assistance will only be received by mail or Online through an appointment line intake.

Applications completed without an appointment will not be accepted for processing.

Step 1: Call the appointment line on:

Mondays 8:30 am to 1:00 pm. and Thursdays 9:00 am to 10:00 am.
(OR UNTIL ALL APPOINTMENTS HAVE REACHED WEEKLY CAPACITY)

All online applications must be signed/completed and submitted by the appointment date scheduled. Online Applications not completed or received by due date will be denied and a new call to the appointment line will be required. No Exceptions!!

Available below are required additional documents (if applicable) that must be completed along with the application.

Additional LIHEAP Documents:

• Self-Declaration Form - No Income in past 30 days of occupants over the age of 18.
• Notice Regarding Collection of Social Security Numbers form.
• Utility Bill Agreement Form - Accounts not listed in applicant name or any adult member inside the home.
