Step Three

Once the client has a confirmed appointment, an application can be submitted through our
LIHEAP Application for Service. Applications can be requested for Mail-out or DropBox pickup during the intake phone interview.

Additional Required Documents Needed (If applicable) for submission with online application

Self Declaration Form- No income in past 30 days of household occupants over the age of 18

Notice Regarding collection of Social Security numbers Form

All requested documentation: that is needed to support your request for assistance: valid photo-ID, All social security cards for all household members, proof of Income for the last 30 days.

Proof of having been affected by COVID-19 Pandemic: Applied for unemployment. Letter of furlough from employer, reduction in hours, etc.
